AMENDMENT #1 for MC AGREEMENT #JB-CSG-14-4365-03Y2 between the STATE OF ARIZONA, GOVERNOR'S OFFICE FOR CHILDREN, YOUTH AND FAMILIES and MARICOPA COUNTY GOVERNMENT C. 2.7./5• 0 o9•2•00 Pursuant to section )00.CVI, AMENDMENTS, the following sections of the above referenced Agreement are hereby amended as follows: Section FE, TERM OF AGREEMENT/EFFECTIVE DATE Pursuant to Teints and Conditions, "Term of Agreement/Effective Date, the State of Arizona hereby exercises its option to amend this Agreement with an extension of nine months. The effective date of extension shall be January 1, 2015 through September 30, 2015. Section IV, REPORTING REQUIREMENTS Pursuant to Terms and Conditions, "Reporting Requirements," of the above referenced Agreement, Grantee shall provide quarterly program activity reports to the Grantor within fifteen (15) working days of the last day of the quarter in which services are provided. The reporting schedule is: • January 1, 2015 —March 30, 2015, due April 15, 2015 • April 1,2015 — June 30, 2015, due July 15, 2015 • July 1, 2015 — September 30, 2015, due October 15, 2015 for the fmal report Section V, MANNER OF FINANCING Pursuant to Terms and Conditions, "Manner of Financing," of the above referenced Agreement, The Grantor hereby exercises its option to amend this contract to provide up to $191,669.71 as additional funding available for cost reimbursement to continue your grant program from January 1, 2015 through September 30, 2015. Carryover funds from Fiscal Year 2014 shall not be available during this contract extension period. The Grantee agrees to adhere to the goals, strategies and activities related to Grantee's Contract. The Grantee shall provide $21,296.63 in matching funds to support the continuance of the grant program from January 1, 2015 through September 30, 2015. The Grantee's budget is pending submission to the Grantor for review and approval. These funds shall not be available for reimbursement to the Grantee until the Grantor has approved the submitted budget and budget narrative. Any obligations of expenditures incurred by the Grantee prior to the budget being approved are made at the Grantee's own risk.
