CONTRACT NO: 860-14-020 AMENDMENT NO: 1 AMENDMENT TO CONTRACT Between MAR1COPA COUNTY By and Through DEPARTMENT OF PUBLIC HEALTH and The Arizona Partnership for Immunization (TAP I) The above named contract is hereby amended as specified below: A. Cover Page, line 6, of the original contract shall be deleted and replaced with the following: '`...the 30 th day of June, 2015.'' This is a no-cost contract extension. No additional services are needed at this time. If MCDPH identifies a need during the new contract period a new scope of work and associated budget shall be negotiated between the two parties and a contract amendment shall be executed by the MCDPH Procurement Officer. III. All other terms and conditions of the original contract shall remain in full force and effect.
