AMENDMENT NO. 6 TO FACILITY USE REVOCABLE LICENSE AGREEMENT
BETWEEN
DIGNITY HEALTH d/b/a
CHANDLER REGIONAL MEDICAL CENTER, (“USER”)
AND
MARICOPA COUNTY, BY AND THROUGH THE DEPARTMENT OF PUBLIC
HEALTH, (“COUNTY”)
RECITALS
A. User and County (the “Parties”) are Parties to that certain Facility Use Revocable License
Agreement No. P50136, dated and approved on May 12, 2010 and subsequently
amended on July 23, 2010, August 5, 2011, June 8, 2012, July 17, 2013 and June 2, 2014
by the Maricopa County Board of Supervisors. The Agreement is for the use of County
leased space at 3002 North Arizona Avenue, Chandler AZ 85225.
B. The term of the agreement expires on May 31, 2015.
C. The Parties now mutually desire to amend the Facility Use Revocable License Agreement
to extend the term and provide a new Exhibit “A”.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
considerations, User and County agree to amend Agreement No. P50136 as follows:
1. The Agreement term is hereby extended seven (7) months, commencing June 1, 2015 to
December 31, 2015. This Agreement is subject to termination pursuant to the provisions
of A.R.S. §38-511.
2. Article 1. User Responsibilities, 1.1, replace with attached Exhibit “A”.
3. The effective date of this amendment is the date of execution by the Maricopa County
Board of Supervisors.
4. The foregoing paragraphs contain all the changes made by this amendment. All other
terms and conditions of the original Agreement and Amendments remain the same and in
full force and effect. User and County acknowledge that there is no Real Estate or other
Commissions owing under this Agreement.
