JESUS 0 VARGAS-MIRANDA, being first sworn, upon oath deposes and says:
That on the 12th day of February, 2015, they presented to Maricopa County, for
payment, a claim for services rendered or supplies furnished during the month of
February 2015, said claim being in the sum of Thirty three and 06/100 dollars, ($33.06)
in payment of which the Maricopa County Board Supervisors caused the Warrant
Number 350020428 to be issued on 02/12/2015, drawn on the County's Expense Fund.
Affidavit further says that although he/she was informed and believes that the
above mentioned warrant was regularly issued, the same has never been received, or
has been lost.
WHEREFORE, affiant requests that the Board of Supervisors issue to him/her a
portion of saidAW rant Number 350020428 in the sum of $33.06.

I had to find an affidavit that I could change to a partial payment request. We will have to provide the COB a
copy of the original check issued and a brief explanation for the partial reissue to Jesus. The attached affidavit
needs to be signed and notarized.
