On or about August 7, 2014, the Area Agency on Aging entered into a Contract for Services with Maricopa County to be administered by its Human Services Dept., Contract #2015-26-MCH in an amount of $1,373,172. 2. The purpose of this, Amendment #1 is to increase Area Agency funding for Case Management by $15,000 from $1,373,172 to $1,388,172 for SAIL client data base development in order to meet Area Agency reporting requirements and standards. 3. All terms of the original Contract not amended hereby remain in full force and effect.
