AMENDMENT TO LEASE AGREEMENT
BETWEEN
FIRST UNITED METHODIST CHURCH of MESA, LESSOR
AND
MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor and Lessee are parties to that certain lease Agreement No. L-7454 dated and
approved August 27, 2014 by the Maricopa County Board of Supervisors. The lease is for
premises at 15 East 1st Avenue, Mesa, Arizona, consisting of 775 square feet of leased
classroom space.
B. The term of the lease expires on May 30, 2017.
C. Lessor and Lessee now mutually desire to amend the lease to extend the lease term and
pursuant to Section 2.2 of the Agreement, add one classroom to the Agreement and
amend Exhibit A to include the new space, expand the playground area, add tenant
improvements, and adjust the rent amount.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
consideration, Lessee and Lessor agree to amend Lease No. L-7454 as follows:
1. The term of the lease is hereby extended one (1) year to commence upon execution of
both Parties and expire May 30, 2018. The Parties may extend the term of the agreement
by written mutual consent. This agreement is subject to termination pursuant to the
provisions of A.R.S. 38-511.
2. The Premises is expanded to include Room B as depicted in Exhibit A
3. Lessee shall perform the following tenant improvements to Room B and playground area:
a) Switch existing toddler commode for dual use toddler/adult commode
b) Add two sinks to Room B
c) Add new vinyl composite tile flooring to room A and B
d) Expand the playground area 500 square feet
4. Lessee agrees to pay as base rent, in equal monthly installments, the sums as follows:

7-1-2015 to 6-30-2016 $1,700.00 $20,400.00

7-1-2016 to 6-30-2017 $1,700.00 $20,400.00
7-1-2017 to 5-30-2018 $1,700.00 $20,400.00
4. The effective date of this amendment is the date of execution by the Maricopa County
Board of Supervisors.
5. The foregoing paragraphs contain all the changes made by this amendment. All other
terms and conditions of the original agreement and as modified by subsequent lease
amendments remain the same and in full force and effect.
