SURPRISE

The City of Surprise, hereinafter referred to as "SUBRECIPIENT", and the COUNTY OF MARICOPA,
hereinafter referred to as "COUNTY" by and through its contract administrator, the Human Services
Department (MCHSD), on behalf of the COUNTY hereby enter into this non-financial Amendment No. 1
to the Agreement entered into on July 30, 2014 and agree to:
1. Extend the contract end dates from December 31, 2014 to June 30, 2015.
2. Revise Section III. Work Statement paragraph E. Performance Reporting Goals as follows:

Project Completion date: June 30, 2015
3. Revise Timeline of Activities as follows:

executed in three or more counterparts each of which shall be deemed an original, but all of which together
shall constitute one and the same instruments.
