AMENDMENT No. 1 TO FACILITY USE REVOCABLE LICENSE AGREEMENT
Between
CITY of PHOENIX (“User”)
And
MARICOPA COUNTY (“County”)
RECITALS
A. This is Amendment No. 1 to Facility Use Revocable License Agreement No. P-50195
dated and approved on April 11, 2012 by the Maricopa County Board of Supervisors (the
“Agreement”). The Agreement is for use of County space at 19401 North cave Creek
Road, Phoenix, AZ 85006.
B. The term of the Agreement expires April 30, 2015.
C. The Parties now mutually desire to amend the Agreement to create a new term, remove
Article 17 and revise contact information.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing recitals and other valuable
consideration, User and County agree to amend Agreement No. P-50195 as follows:
1. The Agreement term is hereby extended two (2) years, to commence May 1, 2015 and
expire April 30, 2017. The Agreement, as amended hereby, is subject to termination
pursuant to the provisions of A.R.S. §38-511.
2. Article 3.1 b) of the Agreement is deleted in its entirety and is replaced with the
following.
Article 3. Counties Responsibilities. 3.1, b) Provide User with a County
administrative contact person is. County’s administrative contact person is:

3. Article 17. Verification Regarding Compliance with Arizona Revised Statutes §§ 35-
391.06 and 35-393.06 Business Relations with Sudan and Iran. Article 17 is removed
from the Agreement in its entirety.
4. The effective date of this Amendment is the date of execution by the Maricopa County
Board of Supervisors.
5. The foregoing paragraphs contain all the changes made by this Amendment. All other
terms and conditions of the original Agreement remain the same and in full force and
effect.
