AMENDMENT NO. 4 TO FACILITY USE AGREEMENT
BETWEEN
DIGNITY HEALTH, d/b/a
CHANDLER REGIONAL MEDICAL CENTER, ("USER")
AND
MARICOPA COUNTY, BY AND THROUGH THE
DEPARTMENT OF PUBLIC HEALTH ("COUNTY")
RECITALS
A. User and County (collectively, the "Parties") are Parties to that certain Facility Use
Agreement No. P50108 dated April 4, 2008 and approved by the Maricopa County Board
of Supervisors and subsequently amended December 17, 2008, January 10, 2010 and
June 19, 2013 (the "Agreement"). The Agreement is for use of County leased space at
3002 N. Arizona Avenue, #13, Chandler, AZ 85225.
B. The term of the Agreement expires March 31, 2015.
C. User and County now mutually desire to amend the Agreement to extend the term and
vaccination schedule, and, expand the definition of "Services" and "Recipients".
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
considerations, User and County agree to amend Agreement P50108 as follows:
1. The term of the Agreement is hereby extended one (1) year, commencing April 1, 2015
and expiring on March 31, 2016. Unless terminated earlier by either Party by giving a
thirty (30) day written notice of termination, the Agreement will automatically renew for
one (1) additional year, expiring March 31, 2017. This Agreement is subject to
termination pursuant to the provisions of A.R.S. §38-511.
2. "Services" is redefined as follows:
Free vaccinations to infants and children, free flu vaccinations to all adults and
children and vaccine at hospital cost with no administration fees to adults
including a discount for students enrolled in a school program leading to a
healthcare degree/certification.

3. "Recipients" is redefined as follows:
The County's children of low income families, adults with decreased accessibility
and or financial acumen to obtain recommended vaccines and adult students
seeking required vaccines to complete a degree in healthcare.
4. Update Exhibit "A" to reflect the 2015-2017 vaccination dates and times.
5. The effective date of this amendment is the date of execution by the Maricopa County
Board of Supervisors.
6. The foregoing paragraphs contain all the changes made by this amendment. All other
terms and conditions of the Agreement remain the same and in full force and effect.
