Wellness Premium Incentives
Proposed changes to the Maricopa County Employee Wellness Premium Incentives:
Non-Tobacco Use:
Employees will receive a reduction in their ongoing medical premium of up to $20.00 per
month if they and their covered dependents enrolled in a County-sponsored medical plan do
not use tobacco products. In order to qualify for rate incentive, employees must pass an
approved screening to detect tobacco use. If they have successfully passed the screening for
tobacco use in the past, employees will not need to be screened again for FY2015-16.
Employees will only be eligible for this premium incentive if they have completed both the
Biometric Screening and the Health Assessment.
Biometric Screening and Health Risk Assessment:
Employees will receive a reduction in their ongoing medical premium of up to $40.00 per
month if they complete both the Biometric Screening and the Health Assessment Self-Exam
offered by the Maricopa County Benefits Department or through an approved sub-contractor.
