Project Name: Improving Quality of Life for Elders, Grandparents and the Children they are Raising, and
Caregivers
In 200 words or less, please provide a brief description of the project submitted to the
12% SRPMIC Grant program.
Salt River Pima-Maricopa Indian Community's generous investment will lead to improved health and
well-being for thousands of people, as follows:
Grandparents Raising Grandchildren: Duet will support grandparents and the children they are raising
through support groups, respite, group activities, workshops, legal guidance, and information. SRPMIC
funds will help 700+ children stay out of foster care.
Family caregivers: Duet assists people caring for aging loved ones through support groups, information
and referrals, educational workshops, and respite. Duet will assist at least 400 caregivers through
SRPMIC's generosity.

Congregational Health: Duet helps congregations develop health programs by training and supporting
faith community nurses. SRPMIC funds will lead to over 65,000 health interventions, including
screenings, health education, referrals, or health counseling, through 75 participating congregational
health programs.
Homebound Adults: Duet will help elders and adults with disabilities remain living in their own homes.
Duet will pair these "neighbors" who no longer drive with volunteers who will provide transportation to
medical appointments, grocery shopping; visiting, home repairs; and computer and paperwork
assistance. Over 775 people will receive 45,000+ hours of service.
In addition, Duet would be pleased to offer workshops for grandparents, caregivers, and congregations
interested in launching health programs at locations selected by SRPMIC.
Beginning Date of Project: July 1, 2014 End Date of Project: June 30, 2015
Geographic Area to be served: Services will be available throughout Maricopa County for caregivers,
grandfamilies, and congregations seeking to develop health promotion. Duet's services for homebound
adults are provided in Scottsdale, Fountain Hills, Phoenix, Glendale, and Peoria.

people in need, improving quality of life while also saving dollars for taxpayers by helping individuals
remain living in their own homes.
If the Amount of funding received is less than the amount needed for the project, do you have other
funds to complete the project? (Please Explain):
Duet sustains its programs with funding from the Area Agency on Aging, Region One; grants
from government, corporate, and founding entities; individuals; sponsorships; special events;
and in-kind donation of office and meeting space. Equally important to Duet's sustainability is
its demonstrated ability to inspire others, model good governance, prioritize goals and
objectives, and make decisions to achieve the organization's mission. The staff has an
exemplary reputation in the nonprofit "aging community" network, serving on boards and task
groups. For over thirty years Duet's Board of Directors and executive leadership have
represented effective and efficient use of the organization's human, financial, and material
resources, confirming Duet's ability to assess and respond to both internal and external
changes. Duet is poised to continue its leadership role in providing quality, cost-effective, and
compassionate help to homebound elders and adults with disabilities, caregivers, faith
communities, and grandparents raising grandchildren. That said, generous community support
is essential to make it possible for Duet to offer its free-of-charge, high quality services.

2013 annual report

We are most grateful to all partners in service who made it possible for Duet to provide life-changing support to thousands
of people in 2013. Generous gifts of time and dollars multiplied as individuals received compassionate help that in turn
saved millions of dollars for taxpayers.
Because people are living longer and our aging population continues to grow, increasing numbers of people need Duet’s
services. For example, the US Census Bureau’s American Community Survey estimates that 69,341 people in our service
area have difficulty with transportation and shopping – Duet’s two most frequently requested volunteer services. Numbers
are also staggering for caregivers, grandparents raising grandchildren, and people needing health promotion services.
Efforts are underway to build capacity and increase services. Duet was honored to be selected as one of six nonprofits
by the Virginia G. Piper Charitable Trust to participate in ATLAS, their two-year capacity-building initiative. Board and
staff members are working with determination to bolster our infrastructure so that vitally needed services will flourish.
What more will it take to serve ever-growing numbers of people needing Duet’s high quality, cost-effective health and
aging services? Greater awareness is key. Please inspire others to volunteer, donate, and request help by sharing your
story of involvement. Your networks may include family, colleagues, faith communities, neighbors, businesses, friends,
and others. Please also consider increasing your financial support. Your gifts of time and dollars will move our cause
closer to achieving our vision of fully meeting the demand for Duet’s vital services.
We cherish your partnership.
