Dear Taxpayer:
This is in response to your Dec. 05, 2012, request for information
regarding your tax-exempt status.
Our records indicate that you were recognized as exempt under
section 501(c)(03) of the Internal Revenue Code in a determination
letter issued in MARCH 1965.
Our records also indicate that you are not a private foundation within
the meaning of section 509(a) of the Code because you are described in
section(s) 509(a)(1) and 170(b)(1)(A)(vi).
Donors may deduct contributions to you as provided in section 170 of
the Code. Bequests, legacies, devises, transfers, or gifts to you or
for your use are deductible for Federal estate and gift tax purposes
if they meet the applicable provisions of sections 2055, 2106, and
2522 of the Code.
Please refer to our website www.irs.gov/eo for information regarding
filing requirements. Specifically, section 6033(j) of the Code
provides that failure to file an annual information return for three
consecutive years results in revocation of tax-exempt status as of
the filing due date of the third return for organizations required to
file. We will publish a list of organizations whose tax-exempt
status was revoked under section 6033(j) of the Code on our website
beginning in early 2011.

Project Name: Freestone Rehabilitation Activity and Senior Center
In 200 words or less, please provide a brief description of the project submitted to the
12% SRP1V1IC Grant program.
Marc has initiated a capital campaign drive for the construction of a community day services and
retirement center for our aging clientele with severe disabilities. The project's intent is to develop
leisure, active living, social and cultural events for seniors with disabilities throughout their
communities. Advancements in medical technology that have increased the life expectancy of adults
with severe disabilities (developmental and behavioral health) prompts the urgency for this project.
Twenty years ago, we were serving 15 individuals over the age of 55; today Marc serves almost
2,000 over the age of 55. This trend underscores the reality that people with severe disabilities will
now outlive their parents, and most families do not plan for the aging of their adult children
relative to guardianship, benefits, resources and care. This project will allow Marc to establish day
activity and retirement programs that include benefit planning. To-date Marc has raised
approximately Si million of its $1.7 million campaign goal. Marc has the guaranteed ongoing
programmatic funds necessary to offset all operational costs once the building is constructed. On
an annual basis, Marc serves approximately 100 Native Americans and 9,000 children and adults

with severe disabilities. Construction began in January and should be completed by November
2014 (see attached rendering).
