I. The above named contract is hereby amended as specified below:
A. Per Section I, Article III, Special Provisions, 1. Compensation for the budget
period of January 1, 2015 through December 31, 2015 will be a fixed rate in the
amount of $5,000.
B. The contract period is January 1, 2015 through December 31, 2015,
All other terms and conditions of the original contract shall remain in full force and
effect.
