Contract No:ADHS12-030680 Amendment No: 4 Tracey Thomas
1. Pursuant to Terms and Conditions, Provision 4, Grant Amendment, Scope of Work, Provision A, Background is
hereby revised and replaced with the following language:
The Arizona Department of Health Services (ADHS), Bureau of Nutrition and Physical Activity (BNPA) with the
Arizona Department of Economic Security (DES), Family Assistance Administration (FAA) provide a program called
the Arizona Nutrition Network (AzNN) The purpose of the AzNN is to provide nutrition education to Arizona’s
Supplemental Nutrition Assistance Program (SNAP) participants and those who are eligible (eligibles) in an effort to
improve healthy lifestyle choices.
The AzNN is funded with Supplemental Nutrition Assistance Program Education Program (SNAP–Ed) dollars
through the United States Department of Agriculture (USDA). USDA, DES, ADHS, and AzNN work together by
focusing this funding to encourage healthful food and lifestyle choices among SNAP participants and eligibles. The
AzNN will contract with a group of public and private organizations committed to working together to improve the
nutrition education, physical activity and obesity prevention of people living in Arizona.
In late 2010 congress passed the Healthy Hunger Free Kids Act (HHFK). The provision moved AzNN into the
Nutrition Education and Obesity Prevention (NEOP) Grant program. NEOP maintains the USDA’s commitment to
providing SNAP-Ed to low-income populations, while providing a focus for the program on the issue of obesity. The
four (4) key aspects of the provision are:
1. Promotion of healthy food choices and consistency with the most recent Dietary Guidelines for Americans.
2. Provision of coordinated services to participants in all the Federal food assistance programs and other low-
income persons.
3. Coordination and collaboration among Federal agencies and stakeholders, including the public health
community.
4. Requirement that activities be evidence-based and outcome driven, providing for accountability and
transparency through state plans and collaboration among stakeholders.
The AzNN will provide grants with individual agencies to provide nutrition education and obesity prevention
programming within their communities. Each Grantee will implement a specific component(s) of the AzNN plan
based on their ability to provide services and the needs of their community. Although all activities must follow
the Supplemental Nutrition Assistance Program Education (SNAP-Ed) Plan Guidance, not all programs will
perform the same activities or functions.
Nutrition messages used by the Grantees reflect the current USDA Dietary Guidelines for Americans and the
USDA Food Guidance System. Grantees are required to utilize nutrition education curriculum/materials
selected for this program by the AzNN.
2. Pursuant to Terms and Conditions, Provision 4, Grant Amendment, Scope of Work, Provision B, Objective is
hereby revised and replaced with the following language:
The USDA, Food and Nutrition Service (FNS) provide guidance to States that encourages the use of the most
effective nutrition education tools and strategies available. The SNAP-Ed Guidance also helps States develop a
clear, comprehensive document for implementing its State plan. FNS encourages States to focus on the following
three (3) key SNAP-Ed behavioral outcomes when assessing needs, developing objectives and outcomes:
1. Make half your plate fruits and vegetables, at least half your grains whole grains, and switch to fat-free or
low-fat milk and milk products;
2. Increase physical activity and reduce time spent in sedentary behaviors as part of a healthy lifestyle; and
