Yes (I:: No The MCDPH's Healthcare for the.Homeless Program (HCH) provides medical, behavioral, and dental care to homeless individuals from a grant received by the Federal Health Services Resource Administration, Health and Human Services Division. The dental care the grant required to provide to the homeless, requires the dental provider to be in very close proximity to HCH because of the lack of transportation amoung homeless individuals as well as their transient lifestyle. . . SINGLE SOURCE CATEGORY (Check applicable box, attach documentation or provide explanation below.) Maricopa County Procurement Code MC1-351 states; "Single source" means an acquisition where, competition is impractical because of standardization, warranty, or other factors as approved by the Board of Supervisors. NOTE: The following are unlikely to be sufficient single source justifications: • Personal or agency preference for a product or vendor • Agency perception that the vendor is the best qualified (this should be determined through a competitive process) • Lack of agency planning resulting in limited time to conduct a competitive procurement • Special incentive or deal offered (can be assessed in open and competitive solicitation) • Agency convenience ' Software license renewals, additions, or upgrades available (--) Grant specified (supporting documents attached) C) from only one source r.-- Brand compatibility available from only one source (no other C Mailing lists, subscriptions or media advertising ' distributors) Cs; Warranty voided if service provided by other contract vendor c- Other proprietary situation (;) Other (Provide written explanation below) THIS PROCURMENT IS A SINGLE SOURCE BECAUSE (attach additional page if needed): Central Arizona Shelter Services (CASS) operates on the same campus as HCH located at 12th Avenue & Madison in downtown Phoenix. Because of this, they are the only viable option for a provider. In addition, CASS offers many other amenities to the population so they have developed a strong rapport with the residents located here. The entire campus serves as a one-stop shop for many of the individuals. CASS is also open during the same hours as the HCH clinic, making it convenient for not only the HCH clients but the staff as well. In addition, the amount of time and effort it would take to revert all the current patient files and records to another provider would be not only be cost prohibitive but could destroy the viability of the current program. The dental provider would not only need to be in close proximity to the homeless population located at 230 South 12th Avenue, but they would also need to be culturally sensitive to the need of the homeless population. Many of homeless population are not well-groomed or bathed which makes them less desirable in a private dental setting. SEARCH A search was conducted consisting of (check all that apply) RESULTS .
