Action Requested:
Approve and authorize the Chairman of the Board of Supervisors to execute Amendment No. 1, a
renewal Lease Agreement No. L-7441 with UMOM New Day Centers, Inc. and Maricopa County by and
through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) for the County’s
use of space (three exam rooms) at the UMOM Campus located at 3333 E. Van Buren, Phoenix, AZ for
the purpose of providing primary care to the residents of the UMOM Campus and the homeless of
Maricopa County. This Agreement is a no cost agreement. The term of this lease agreement
commences on November 1, 2013 expiring on October 31, 2014.

This Lease Agreement L-7441 was endorsed by the County’s Facility Review Committee during June of
2012.

AMENDMENT No. 1 to LEASE AGREEMENT
BETWEEN
UMOM NEW DAY CENTERS, Inc. (LESSOR)
AND
MARICOPA COUNTY (LESSEE)
RECITALS
A. WHEREAS Lessor and Lessee are parties to that certain Lease Agreement No. L-7441
dated and approved July 25, 2012 by the Maricopa County Board of Supervisors; and,
B. WHEREAS The Lease is for premises at 3333 E Van Buren, Phoenix, AZ 85008
consisting of space to provide primary care to the residents of UMOM Campus and the
homeless of Maricopa County; and,
C. WHEREAS The term of the lease expires October 31, 2013; and,
D. WHEREAS Lessor and Lessee now mutually agree to amend the Lease to create a new
lease term.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
considerations, Lessor and Lessee agree to amend Lease No. L-7441 as follows:
I. The term of the lease is hereby extended one (1) year commencing on November 1,2013
and expiring on October 31, 2014. This Agreement is subject to termination pursuant to
the provisions of A.R.S. 38-511.
2. This amendment shall not be effective until it is executed by the Chairman of the
Maricopa County Board of Supervisors.
The foregoing paragraphs contain all the changes made by this amendment. All other
items and conditions of the original agreement remain the same and in full force and
effect.
