AMENDMENT No. 1 TO FACILITY USE REVOCABLE LICENSE AGREEMENT
Between
THE CRISIS NURSERY, INC (“User”)
And
MARICOPA COUNTY (“County”)
RECITALS
A. User and County (the “Parties”) are Parties to that certain Facility Use Revocable License
Agreement No. P-50150 dated and approved on February 18, 2011 by the Maricopa
County Board of Supervisors (the “Agreement”). The Agreement is for use of County
space at 1645 E. Roosevelt Street, Phoenix, AZ 85006.
B. The term of the Agreement expires September 30, 2014.
C. The Parties now mutually desire to amend the Agreement to create a new term, update
County administrative point of contact, remove Article 17, and provide a new Exhibit
“A”.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
considerations, USER and County agree to amend Agreement No. P-50150 as follows:
1. The Agreement term is here by extended three (3) years, to commence October 1, 2014
and expire September 30, 2017. This Agreement is subject to termination pursuant to the
provisions of A.R.S. §38-511.
2. Article 1. User Responsibilities. 1.1, replace with attached Exhibit “A”.
3. Article 1. User Responsibilities. 1.3, User’s administrative contact person for the
County is:

4. Article 3. Counties Responsibilities. 3.1, b) Provide User with a County

5. Article 17. Verification Regarding Compliance with Arizona Revised Statutes §§ 35-
391.06 and 35-393.06 Business Relations with Sudan and Iran. Article 17 is removed
from the Agreement in its entirety.
6. The effective date of this Agreement is the date of execution by the Maricopa County
Board of Supervisors.
7. The foregoing paragraphs contain all the changes made by this Amendment. All other
terms and conditions of the original Agreement remain the same and in full force and
effect.
