I. The above named contract is hereby amended as specified below:
A. Change the following items under Section 6.28 Incorporation of Documents,
6.28.1, Exhibit A, Pricing, on Page 15, Dental Benefits Proposal as:
1. Proposed Effective Date: March 1, 2014 and;
2. Annual Maximum Benefits: $2,500
II. All other terms and conditions of the original contract and its amendments shall
remain in full force and effect.
