AMENDMENT TO AFFILIATION AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
YALE UNIVERSITY
For The Rotation of Students
The above named agreement is hereby amended as specified below.
A. The original contract shall be amended to change the term of this Affiliation
Agreement to July 1, 2014 through June 30, 2019 as indicated in the first
paragraph and also in Paragraph 5.A.
B. Paragraph 6A of the Agreement is amended to read: "YALE and MCDPH shall
not discriminate against any individual on the basis of age, race, religion, creed,
sex, national origin, handicap, disability, sexual orientation or veteran status. To
the extent applicable to this Agreement, both parties agree to comply with all
state and federal employment laws, rules, regulations and executive orders,
including ,Executive Order 2009-09.
All other terms and conditions of the original Agreement shall remain in full force and
effect.
