AMENDMENT TO MEMORANDUM OF UNDERSTANDING

MARICOPA INTEGRATED HEALTH SYSTEMS, dba MARICOPA HEALTH PLAN
AND
MARICOPA COUNTY BY AND THROUGH ITS DEPARTMENT OF PUBLIC HEALTH
dba MARICOPA COUNTY HEALTH CARE FOR THE HOMELESS
Purpose: To clarify the contractual relationship between Maricopa Integrated Health Systems dba
Maricopa Health Plan.
Issue: Maricopa Integrated Health Systems dba Maricopa Health Plan and University Physicians
Healthcare dba University Physicians Health Plans entered into a joint venture agreement for Maricopa
Care Advantage. Effective January 1, 2011 University Physicians Health dba University Physicians Health
Plans sole ownership of Maricopa Care Advantage.
Resolution: This Memo of Understanding serves as a notification of this change and to ensure the
proper name is documented in your Agreement.
Recitals: University Physicians Healthcare dba University Physicians Health Plans has entered into a
contract with a Medicare Advantage Organization to provide services to Medicare beneficiaries who are
Members of a Medicare Advantage Special Needs Plan.
All other terms and conditions of this contract remain in effect.
