AMENDMENT No. 1 TO FACILITY USE REVOCABLE LICENSE AGREEMENT
BETWEEN
CHILD CRISIS CENTER
AND
MARICOPA COUNTY
This Amendment No. 1 (“Amendment”) to the Facility Use Revocable License
Agreement (“Agreement”) No. P50234 by and between Child Crisis Center, an Arizona non-
profit corporation (“Contractor”) and Maricopa County, a political subdivision of the State of
Arizona (“County”), dated and approved on November 7, 2013 by the Maricopa County Board
of Supervisors, is made and entered into by and between the Contractor and the County,
sometimes collectively hereinafter referred to as the “Parties.”
RECITALS
A. The Agreement is for use of County leased space at 635 E Broadway, Mesa, AZ 85204.
B. The term of the Agreement expires on October 22, 2014.
C. The Parties now mutually desire to amend the Agreement to extend the Agreement term
and update Exhibit “A”.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other good and valuable
consideration, the receipt and sufficiency of which are hereby acknowledged, Contractor and
County agree to amend Agreement No. P50234 as follows:
1. The Agreement term is hereby extended approximately two (2) years, to commence on
October 23, 2014 and expire on October 31, 2016. This Agreement, as amended by this
Amendment, is subject to termination pursuant to the provisions of A.R.S. §38-511.
2. Article 2. Contractor Responsibilities. Subsection 2.1, of the Agreement is hereby
amended by replacing Exhibit “A” in its entirety with the Exhibit “A” attached hereto and
incorporated herein by this reference.
3. The effective date of this Amendment is the date of execution by the Maricopa County
Board of Supervisors.
4. The foregoing paragraphs contain all the changes made by this Amendment. All other
terms and conditions of the original Agreement remain the same and in full force and
effect.
