Purpose of Grant a) Describe the proposed program or project The Community Connections program will provide opportunities for inmates released from the Maricopa County Jail to secure safe/sober living accommodations and engage in employment services designed to enhance their ability to be self-sufficient and refrain from further contact with the justice system. Program entry begins with a simple referral from social workers at the Maricopa County Jail facilities. Social Workers will focus on Moderate to High-Risk and High-Need inmates who have participated in treatment and/or cognitive education programs while incarcerated. Identified inmates will meet with a Coninnmity Connections case manager to discuss individual needs within four domains: Housing, Employment, Transportation, Identification. Housing assistance will be offered to inmates who do not have a secure and sober living environment to return to upon release. Based on information provided by the inmate such as preferred geographic area and support needs, the Community Connections case managers will identify options for the inmate to review. Once the participant has identified their sober living residence of choice, the Community Connections case manager will authorize payment on a monthly basis, reassessing the need as the participant becomes employed and nears self- sufficiency. The supportive housing assistance can continue for up to 6 months, with clients working toward self-pay once financial stability has been achieved. Supportive services, such as clothing/uniforms, tools, identification, credentials, GED expenses will be provided to clients on an individual need basis. Transportation assistance, in the form of bus passes, gas cards and/or taxi/commercial transportation services will be provided based on client need, utilizing the least expensive but viable mode of transportation. b) Describe how the proposed program or project satisfies one or more of the "priority areas" identified by the Gila River Indian Community. The Community Connections program satisfies the Public Safety priority area, as the primary objective of the program is to reduce recidivism of Moderate to High-risk offenders leaving the Maricopa County Jail and returning to the community. This program provides case management services to offenders referred by jail staff, with services which include housing, transportation and supportive services. The Community Connections program also impacts the Education and Transportation Rev. 4/2013 Gila River Indian Community Grant Application priority areas through its focus on connecting former inmates with viable employment and education/training programs. Program participants can participate in core employment services such as resume writing and job search workshops. For those who have met basic education requirements and meet the Worforce Investment Act (WIA) program eligibility requirements, funding for intensive level services is available (leveraged resources) which includes credentialed training programs, career guidance and exploration services and more. Transportion assistance is available to all participants while job-seeking and attending training programs, counseling, treatment or probation meetings. c) Identify the needs/problems to be addressed, target population and number of people to be served by the project. Inmates classified as Moderate to High-Risk for recidivating are placed in evidence-based treatment and cognitive education programs which have proven to increase pro-social skills and decrease criminal conduct. Examples of these programs include the ALPHA Program for substance abuse treatment and the Thinking for a Change cognitive education program. With the addition of social workers in the jail, offenders are provided with comprehensive community reentry planning designed to make the transition from jail to community seamless. Currently, inmates participating in evidence-based programming and/or cognitive education programs such as the ALPHA Program recidivate at a rate of 32.5%. The target population for the Community Connections program will be Moderate to High-Risk and High-Needs inmates from the sentenced population (male and female) who have participated in evidence-based treatment programs such a the ALPHA Program and Veteran's program and those inmates from the sentenced population who participate in cognitive education programs such as Thinking for a Change. It is anticipated that a minimum of 90 inmates will be referred and voluntarily participate in the Community Connections program. Alpha Program Referrals 25 Veteran's Program Referrals 20 Thinking for a Change — Sentenced Females Referrals 20 Thinking for a Change — Sentenced Males Referrals 25 Total Referrals 90 Jail staff have identified a lack of secure housing, unemployment and lack of transportation as issues cited by inmates returning to jail for a new offense after the successful completion of evidence based treatment and education programs while incarcerated. Additionally, offenders returning to the community often have little or no funding to acquire permanent state ID or driver's license, social security card, birth certificates, GED or HS diploma and other credentials often required for employment and assistance programs. Although program supports such as evidence-based training and education, partnerships and data sharing with correctional health/behavioral health staff and reentry planning within the jail have made a significant improvement in the incidents of recidivism, we believe that the Community Connections program will reduce the likelihood of new offenses and technical violations of probation which lead to subsequent jail terms for inmates once they have been released. Rev. 4/2013 Gila River Indian Community Grant Application d) Describe the project goals and objectives, and your plan to meet them. The project goals are designed to demonstrate effectiveness of the program. The overall goal is a reduction in recidivism rate (or return to jail) for inmates assessed as Moderate to High-Risk and High-Need. By reducing subsequent offenses and inmate return to the costly jail environment, this program hopes to show results that enhance community safety and reduce the harm caused by repeat offenders. Goal: Decrease recidivism rate of inmates released by Mciricopa County jail and referred to the Community Connections program. Objectives: • Provide case planning services to 90 inmates returning to the community, utilizing Motivational Interviewing and Thinking for a Change techniques • Assist program participants in identification of their challenges and barriers to success • Provide connections to community organizations and supports that address identified barriers to success Performance Expectation: Recidivism rate reduction from average of 51.4% (ALPHA Program comparison group) and 32.5% (ALPHA Program treatment group) Goal: Increase community connection and self-sufficiency qfpro gram participants. Objectives: • Provide case planning services to 90 inmates returning to the community • Provide access to a supportive/sober living residence for 90 Community Connections participants for up to 6 months, transitioning to self-pay and/or independent living as soon as practical • Provide career development and/or employment services to 90 Community Connections participants to increase their self- sufficiency • Provide transportation and supportive services to 90 Community Connections participants to enhance successful reentry and reengagement Performance Expectation: Program participants employment rate will be at or above 80%; 80% of Employed participants will retain employment for 6 months or snore e) Define the project as a new or continuing program. The Community Connections program is a new/pilot program for the Maricopa County Human Services Department. Rev. 4/2013 Gila River Indian Community Grant Application 1) Identify other organizations, partners or funders participating in the project and their roles (see attachments section). The Maricopa County Sheriffs Office is the retelling party and program partner for the Community Connections program. Social workers will identify candidates amongst the Moderate to High-Risk and High-Needs inmate populations, primarily participants in the Alpha Program, Female Sentenced population, Male sentenced population and Veteran's Program. Additionally, social workers will ensure program participants have identification upon release from secure care/jail including state issued ID, driver's license or temporary ID supplied by the jail. The Maricopa County Human Services Department (HSD), through its Workforce Development Division, will provide program participants with core level employment services (workshops, job search) and intensive level employment and career development services to those who qualify for funding under the Workforce Investment Act. HSD, through its Community Services Division, will provide homeless prevention/housing/utility assistance to program participants who qualify for services under the Community Action Program (CAP). These services can assist with utility deposits and monthly charges, rental home move-in fees and first month's rent. Assistance is dependent on availability of funds at time of referral to the CAP Program. Indicate any application to and/or awards made by a Tribe other than the Gila River Indian Community for state shared revenues for this and/or any other projects/programs. At this time, no applications have been submitted to the Gila River Indian Community or any other Tribefor this project by the Maricopa County Human Services Department. The Maricopa County Human Services Department has not submitted applications for funding assistance to a Tribe, including Gila River Indian Community, for other projects/programs, h) Provide a timetable for implementation. Once awarded funding, the program implantation plan includes recruitment of case management staff with experience working with justice system partners, such as MCSO and Adult Probation. This process is anticipated to take no longer than two months. Social workers within the Maricopa County jail will be notified that referrals will be accepted as soon as the Community Connections case management positions are filled and staff have received training. It is anticipated that the 45-60 program participants will be identified and begin services within the first 12 months of program operation. The Human Services Client Services contracts will reopen for additional vendor solicitation upon notification of grant award. I have had the opportunity to supervise the Maricopa County Sheriffs Office Transition Services since 2011. During that time, I have identified the need for jail reentry services within the walls of the jails, as well as support for those transitioning back into the community, The high risk to recidivate population that MCSO is currently focusing on,for programming and transition planning still have many needs once these individuals are released from jail. The services that will be offered by the Community Connections Program will support the transition planning done inside the jails and the individual's needs as they enter the community with hopes of being successful. I believe Community Connections is a necessary program that will support criminal justice involved individuals in becoming productive citizens, and in turn make our communities safer.
