Helicopter Flight Services Agreement
Amendment No. 1
Amend Helicopter Services Agreement No. C0950 per ARTICLE 2, TERM, to extend the Agreement for an
additional five (5) years resulting in a new ending date of September 30, 2019.
Also amend Article 12, NOTICES to change the address for notifications as follows:
To Maricopa County: Maricopa County Sheriff's Office

All other terms and conditions of the original Helicopter Flight Services Agreement remain the same.
