As a result, CAWCD has drafted an amendment to extend the term of the License for 5 years. Attached please find the renewal amendment to the license. Sign and notarize two copies and return both to CAWCD. One fully executed original will be returned to MCSO for its files. Insurance must be maintained throughout the term of the License. Please be aware that by extending the license term, the term of your certificate of insurance which applies to this license may need to be extended. Thank you for your attention to this matter. The new termination date will be September 30, 2019. The change described in this amendment is the only change to the license; all other provisions of the license remain the same.
