AMENDMENT 3
The Contract entered into between Maricopa County, hereinafter referred to as the Contractor, and the Director of
the Arizona Department of Corrections, for and on behalf of its Arizona State Prison Complex-Perryville, hereinafter
known as the Department, is hereby amended as follows:
Purpose of Amendment:
1. In Accordance with Section 3- Term of Contract, Paragraph 3.1, this contract is being extended for an additional twelve (12)
months; Effective April 14, 2014 through April 13, 2015.
All other terms and conditions of this Contract shall remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto agree to carry out the terms of this Amendment.
