GRANT AGREEMENT BETWEEN MARICOPA COUNTY AND GUADALUPE COMMUNITY DEVELOPMENT CORPORATION HOME INVESTMENT PARTNERSHIPS PROGRAM ACTIVITY TITLE: HOME Investment Partnerships Program AGENDA NO.: C-22- CFDA NO.: 14.239 DUNS NO.: 015750361 ACTIVITY NUMBER: CH1202 ACTIVITY AMOUNT: $100,000 Amendment No. 2 THIS Amendment No. 2 is entered into by and between Guadalupe Community Development Corporation (Community Housing Development Organization, CHDO), a nonprofit agency, hereinafter referred to as "CHDO", and the COUNTY OF MARICOPA, hereinafter referred to as "COUNTY" to be administered by the Board of Supervisors of Maricopa County through its contract administrator, the Human Services Department (MCHSD), on behalf of the COUNTY. This AMENDMENT is to be fully incorporated and made a part of this Grant Agreement. Whereas, the CHDO and the COUNTY hereby agree to amend the Grant Agreement with an effective date of July 25, 2012 page 1, contract termination date to read: August 1, 2014 And Page 78, Section IV E. ENCUMBRANCE & EXPENDITURE REQUIREMENTS: to read: The CHDO agrees to encumber 100% of the contract amount by June 1, 2014. All funds shall be drawn from the Maricopa County letter of credit by June 30, 2014. This Amendment may be executed in three or more counterparts each of which shall be deemed an original but all of which together shall constitute one and the same instruments.
