January 29, 2014 Dear Maureen, Congratulations! On December 31, 2013, Maricopa County Head Start was awarded a grant from Share Our Strength for $1500 to offer Cooking Matters courses that will reach a minimum of 20 adults by July 30, 2014. This grant is made possible by generous support from Newman’s Own Foundation. By accepting this grant you agree to the following terms and conditions: These funds must be used to support the implementation of Cooking Matters six-week courses as outlined in the eligible use of grant funds in the RFP. Before beginning courses, the Cooking Matters course coordinator, and instructors if applicable, must successfully complete a series of online training modules. They will receive a link to the online training in the first week of January. The course coordinator must complete an online form for each course scheduled and completed. The online training will teach the coordinator how to do this. The course coordinator must administer a paper-based pre- and post-course survey and enrollment form to each course participant to complete before the first class and following the last class. All completed surveys and enrollment forms must be returned to Share Our Strength within one week of completing the course (details to be addressed during online training). After the completion of the grant cycle on July 30, 2014, the course coordinator must complete a satisfaction and feedback survey documenting your experience with the online training and course coordination. If any point during the grant period your organization anticipates that you will not be able to achieve your grant goals prior to July 30, 2014, please contact Share Our Strength as soon as possible through contacting eevancho@strength.org. Share Our Strength will either work with you to revise your strategies and goals, or we will discuss the possibility of returning all or part of the grant. The grantee agrees to notify Share Our Strength of any change in your public charity status. This agreement also gives Share Our Strength permission to use photographs, logos, and other materials you supply in your grant reports to Share Our Strength, without further notice, in press releases and/or publications. By cashing the grant check, which will be mailed after this letter is signed and returned to Share Our Strength, you agree to abide by all conditions described in this letter. If you have any questions about any of the conditions described in this letter, or about your grant in general, please contact Elizabeth Evancho at 202-478-6576 or eevancho@strength.org. We wish you the best of success and look forward to hearing about your progress.
