AMENDMENT to LEASE AGREEMENT
BETWEEN
SANTOS C. NIETO, Sr., LESSOR
AND
MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor and Lessee are parties to that certain lease Agreement No. L-7225 dated and approved
by the Maricopa County Board of Supervisors May 1, 1996 and amended January 17, 2001,
January 18, 2006, and February 23, 2011. The lease is for premises at 9206 South Avenida del
Yaqui, Guadalupe, Arizona, consisting of 1,920 square feet of office and clinic space.
B. The term of the lease expires February 28, 2014
C. Lessor and Lessee now mutually agree to amend the lease to create a new lease term, update
Lessor and Lessee notice address.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable considerations,
Lessor and Lessee agree to amend Lease No. L-7225 as follows:
1 The term of the lease is hereby extended eight (8) months, commencing March 1, 2014 and
expiring October 31, 2014. This agreement is subject to termination pursuant to the provisions
of A.R.S. § 38-511.

4. Lessee agrees to pay as base rent, in equal monthly installments, the sums as follows:

2014 $13.83/sf $2,212.80 $26,553.60 plus rental tax
Maricopa County is responsible for its share of costs for janitorial services and utilities,
estimated at $480.00 per month.
5. The effective date of this amendment is the date of execution by the Maricopa County Board of
Supervisors.
6. The foregoing paragraphs contain all the changes made by this amendment. All other terms and
conditions of the original agreement and as modified by subsequent amendments one, two, and
three remain the same and in full force and effect.
