AMENDMENT NO. 4 TO THE  
CONTRACT 
BETWEEN 
MARICOPA COUNTY  
ADMINISTERED BY ITS HUMAN SERVICES  
DEPARTMENT  
AND 
UMOM NEW DAY CENTERS 
 
I. 
Maricopa County (“County”) administered by its Human Services Department and 
UMOM New Day Centers (“Subrecipient”) entered into a financial agreement 
(“Agreement) on or about September 10, 2024. The current Agreement term is July 
1, 2025, through June 30, 2026. The purpose of the Agreement is for the Subrecipient 
to provide emergency shelter and case management services for individuals and 
families at risk of or experiencing homelessness. The County provided the 
Subrecipient with U.S. Department of Housing and Urban Development (HUD) 
Community Development Block Grant (CDBG) funds under Assistance Listing 
Number (ALN): 14.218 and Emergency Solutions Grant (ESG) funds under 
Assistance Listing Number (ALN): 14.231, for Program Year 2025, Fiscal Year 2026. 
The County and the Subrecipient may be referred to individually as the “Party” or 
collectively as the “Parties.” 
 
II. 
The Parties now agree to modify the Agreement as authorized by Section 1 (General 
Provisions), Paragraph 4.0 (Amendments) of the Agreement as follows: 
 
A. 
The Agreement total funding shall increase by $77,000 to award the Subrecipient 
County General funds, funds to be fully expended by June 30, 2026. 
 
B. 
Revise Contract amount on Page 1 to reflect a new Agreement funding total of 
$614,792.84 
 
C. 
Add Federal Award Identification Numbers (FAIN) and Award Dates to Page 1 of 
the Agreement:

FAIN / AWARD DATE: 14.231: E25UC040501 awarded 9/16/2025 
 
D. 
Extend the Agreement Expiration Date on Page 1 through June 30, 2027, for 
the budget period of July 1, 2026, through June 30, 2027 (Fiscal Year 2027). 
Costs incurred in the FY27 budget period shall only be eligible for 
reimbursement under this Agreement subject to County’s receipt of funding, 
once funds have been added to the Agreement by Amendment.

2.1 
The County shall provide total amount not-to-exceed Six Hundred 
Fourteen Thousand Seven Hundred Ninety-Two Dollars and 
Eighty-Four hundredths ($614,792.84) subject to the terms of this 
Agreement and availability of funds. Funding is provided by U.S. 
Department of Housing and Urban Development (HUD) Community 
Development Block Grant (CDBG), Emergency Solutions Grant 
(ESG) funds, and Maricopa County General funds for the years 
identified in 2.0 Budget. This Agreement amount constitutes the 
County’s entire participation and obligation in the performance and 
completion of all work to be performed under this Agreement.

Funding for this Agreement is through the following published 
programs in the Federal Register by the executive departments and 
agencies of the Federal Government of the United States under 
Assistance Listing Number (ALN): 
$409,047 - 14.218 Community Development Block Grant,  
$128,745.84 - 14.231 Emergency Solutions Grant, 
$77,000 – Maricopa County General Funds

Section II above contains all the changes to the Agreement made by this Amendment 
No. 4. The Agreement is amended to incorporate the changes contained in this 
Amendment No. 4. All other terms and conditions of the Agreement remain in full 
force and effect as executed by the Parties. This Amendment No. 4 is subject to and 
incorporates the provisions of A.R.S. § 38-511.

IV. 
The Parties have authorized the undersigned to execute this Amendment No. 4 on 
their behalf and it shall be effective upon approval and signature by both Parties.
