AMENDMENT NO. 3 TO THE  
CONTRACT 
BETWEEN 
MARICOPA COUNTY  
ADMINISTERED BY ITS HUMAN SERVICES  
DEPARTMENT  
AND 
NATIVE AMERICAN CONNECTIONS, INC 
 
I. 
Maricopa County (“County”) administered by its Human Services Department and 
Native American Connections, Inc. (“Subrecipient”) entered into a financial 
agreement (“Agreement) on or about August 28, 2024. The current Agreement term 
is July 1, 2025, through June 30, 2026. The purpose of the Agreement is for the 
Subrecipient to provide emergency shelter and case management services for 
individuals and families at risk of or experiencing homelessness. The County 
provided the subrecipient $206,639  in Fiscal Year 2026 Emergency Solutions Grant 
(ESG) funding through an Intergovernmental Agreement (IGA) with Arizona 
Department of Economic Security (DES) under IGA DI23-002395/ C-22-23-107-X-
00. The County and the Subrecipient may be referred to individually as the “Party” or 
collectively as the “Parties.” 
 
II. 
The Parties now agree to modify the Agreement as authorized by Section 1 (General 
Provisions), Paragraph 4.0 (Amendments) of the Agreement as follows: 
 
A. 
Add Federal Award Identification Number (FAIN) and Award Date to Page 1 of 
the Agreement:  
 
FAIN / AWARD DATE: 14.231: E25UC040501 awarded 9/16/2025 
 
B. 
Extend the Agreement Expiration Date on Page 1 through June 30, 2027, for 
the budget period of July 1, 2026, through June 30, 2027 (Fiscal Year 2027). 
Costs incurred in the FY27 budget period shall only be eligible for 
reimbursement under this Agreement subject to County’s receipt of funding, 
once funds have been added to the Agreement by Amendment. 
 
C. 
In Section 3 (Work Statement) revise and replace the County Point of Contact 
in Paragraph 3.0 (Notices):

III. 
Section II above contains all the changes to the Agreement made by this Amendment 
No. 3. The Agreement is amended to incorporate the changes contained in this 
Amendment No. 3. All other terms and conditions of the Agreement remain in full 
force and effect as executed by the Parties. This Amendment No. 3 is subject to and 
incorporates the provisions of A.R.S. § 38-511. 
 
IV. 
The Parties have authorized the undersigned to execute this Amendment No. 3 on 
their behalf and it shall be effective upon approval and signature by both Parties.
