Description of the Firm Offering Professional Services. Health Resources in Action (HRiA) is pleased to submit this proposal to the Maricopa County Department of Public Health (MCDPH) to provide the health department leadership with a series of products and strategies for strengthening its ability to partner with the health care sector as you mutually seek to achieve the Triple Aim of health reform: Improved Patient Care, Improved Population Health Outcomes and Lowered Health Care Costs. HRiA is a 501(c) 3 national nonprofit public health and medical research organization located in Boston, MA serving governmental, community, health care, and philanthropic organizations across the United States. Founded in 1957, HRiA works to help people live healthier lives and create healthy communities through policy, prevention, health promotion, and research. This broad mission statement has remained consistent throughout HRiA's history and is reflected in the organization's various services and competencies, including research, assessment, and evaluation; strategic planning and organizational effectiveness; public policy and systems change; training and capacity building; youth development, and health communications. With a strong focus on the multi-faceted issues facing vulnerable populations and a deep understanding of health disparities, HRiA has earned a national reputation as a leader in promoting healthy communities. 3 . Approach to Services Through a number of components and strategies, HRiA's consultant team will produce five overarching deliverables based on MCDPH's articulated aims: a) Communications materials and strategies for collaborating with the health care sector in six major areas • Factsheets describing the rationale and approaches for adopting best practices for clinical and community/public health partnerships in six areas. One of the six areas will address the overall value of prevention and population health strategies. The remaining five factsheets will address healthy eating, active living, diabetes, asthma, and cardiovascular disease. These topics may change based on the economic and epidemiological data available as this project unfolds. • INFOGRAPHIC s that will visually represent the key concepts explicated in each of the six areas identified above • Webinars, will be presented to various audiences (e.g. MCDPH staff, payers, providers, etc) via HRiA's Community Health Training Institute platform. The topics will be determined by MCDPH, with a maximum of six presentations. The objective of these webinars will learn together about cost effective practices for addressing key health challenges, such as asthma. 1 Oral Health System Assessment • Technical Assistance Consulting on communications strategies for the above deliverables b) Best practice report will be developed to address two major public health priority issues not currently being addressed by MCDPH: Elder Falls Prevention and Prescription Drug Abuse • FRIA will research the published and gray literature to provide background on Elder Falls Prevention and will identify evidence-based best practices for addressing them. In addition, MCDPH will provide background research to HRiA on Prescription Drug Abuse, and then HRiA will develop an Operational Action Plan describing the "nuts and bolts' ) of getting the work done in a large, urban local health department. • Three model health departments, experienced in addressing these two topics, will be interviewed to describe how they are implementing these prevention programs through their health departments, and whether they are showing results. Short case studies will be written up. • The literature and qualitative data will be synthesized into a report that will be used by MCDPH to inform potential program development in these two areas. It will incorporate a practical Action Plan for Prescription Drug Abuse. c) Healthy Worksites Toolkit. HRIA will identify and/or create evidence based or evidence-informed worksite wellness resources that provide practical guidance for small, medium and large employers to assist them in implementing worksite wellness initiatives in up to five topic areas (physical activity, healthy eating, prescription drug abuse, tobacco, and biometric screenings/immunizations). These topics may change based on the employer and employee data that becomes available during the assessment phase of this project. Included in the toolkits (one toolkit for each topic) will potentially include messaging/communication strategies and/or information on existing campaigns to entice employees to participate in wellness initiatives, sample policy language, facilitators to success, ways to address potential challenges that employers may face to implement these, etc. d) Technical Assistance for Assisting Key MCDPH Staff to move their work upstream • While the format is to be determined, at least two staff from HRiA will conduct a brief assessment of key departments within MCDPH, and organize /lead a full day retreat that will assist staff to analyze their current work and identify opportunities for them to not only enhance their partnerships with the health care sector, but also identify best practice strategies for moving their programs into more impactful and sustainable approaches that utilize policy, environmental and systems change strategies. 5. Timeline. The project is expected to begin the first week of January 2014 and be completed by June 30, 2014 6. Project Assumptions: a) MCDPH staff will identify their five priority areas to be researched by January 6 th, 2014 b) MCDPH economists will research and communicate their findings to HRIA staff on costs associated with the five or six priority chosen areas to research c) Up to five topics can be researched for the healthy worksites toolkit with the resources available. These two topics will be identified by January 13 th, 2014 by MCDPH. MCDPH will be responsible for uploading the information to its website d) MCDPH will take responsibility for publicity and recruiting for the webinars 2 Oral Health System Assessment e) MCDPH staff will be available on a bi-weekly basis for planning calls; In January weekly calls may be required. f) MCDPH will respond in a timely fashion regarding requested feedback on products.
