(2013 – 2014 Utility Assistance Programs)

The Independent Contract Agreement dated June 19, 2013, and amended on October 25, 2013,
between Arizona Community Action Association (ACAA) and Maricopa County,
administered by its Human Services Department (hereinafter “Contractor”) to conduct
application intake and eligibility determination for repair and replacement of eligible appliances
is hereby amended as follows:
Purpose of the Amendment:
1. To allocate additional URRD funds in the amount of $100,000.00 ($92,000.00 for direct
service and $8,000.00 for program delivery) in order to provide additional repair and
replacement services to eligible families residing in Maricopa County.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources:
URRD Fund Source: Increase direct service amount dollars from $143,640.74 to $233,640.74,
and program delivery dollars from $11,491.26 to $19,491.26. The total amount of URRD funding
increased from $155,132.00 to $255,132.00.
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in
place for the duration of the original Agreement.
