AMENDMENT TO
CONTRACT

MARICOPA COUNTY
By and Through
DEPARTMENT OF PUBLIC HEALTH
and
DELTA DENTAL OF ARIZONA FOUNDATION
The above named contract is hereby amended as specified below:
A. Cover Page, line 3, Contract Amount is changed to an aggregate total not-to-
exceed (NTE) $153,000 (increased by the NTE amount of $44,000 for the budget
period July 1, 2013 through June 30, 2014).
B. Cover Page, line 6, the Expiration Date has been extended to June 30, 2014.
C. One final project report is due at the completion of the project (no later than June
1, 2014).
All other terms and conditions of the original contract and any amendments shall
remain in full force and effect.
