AMENDMENT TO FACILITY USE/REVOCABLE LICENSE AGREEMENT
BETWEEN
KEOGH HEALTH CONNECTION, ("USER")
AND
MARICOPA COUNTY, ("COUNTY")
RECITALS
A. County and User are parties to that certain Amendment To Facility use/Revocable
License Agreement No. P-50183 approved by the Maricopa County Board of
Supervisors on November 30, 2011 and amended November 28, 2012. The
License is for use of County leased premises at 1260 South 7 111 Avenue, Phoenix,
AZ 85007.
B. The term of the License expires on November 30, 2013.
C. County and User now mutually desire to amend the license to create a new term.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable
considerations, County and User agree to amend License No. P-50183 as follows:
1. The term of the License is hereby extended one (1) year, to commence on
December 1,2013, and to expire on November 30, 2014. This Agreement is
subject to termination pursuant to the provisions of A.R.S. 38-511.
2. The effective date of this amendment is the date of execution by the Maricopa
County Board of Supervisors.
3. The foregoing paragraphs contain all the changes made by this amendment. All
other terms and conditions of the original agreement and amendments thereto
remain the same and in full force and effect.
