Terms and Conditions Failure to comply with the special remarks and condition(s) may result in a draw down restriction being placed on your Payment Management System account or denial of future funding. Program Specific Term(s) 1. This award provides outreach and enrollment supplemental funding for the period July 1, 2013 through June 30, 2014. As such, a portion of these funds are being provided for use in the grantee's upcoming FY 2014 budget period. In order to use this funding in the upcoming budget period as planned, the appropriate amount must be shown as un-obligated (UOB) on line 10.h of the Annual Federal Financial Report (FFR), SF 425. In addition, a Prior Approval Request to carry over these funds must be submitted through EHBs immediately following the FFR submission. Please consult the Grants Management Specialist for questions regarding submission of the FFR and/or Prior Approval Requests to carry over UOB funds. Funds will be awarded in FY 2014 to align with remaining months in the grantee's FY 2014 budget period. 2. The grantee will be required to report on progress toward the 0/E goals outlined in the application via a quarterly progress report (QPR) to be submitted through the HRSA Electronic Handbook (EHB), HRSA will provide additional guidance regarding specific requirements and deadlines through separate resources that will be available at http://bphc.hrsa.gov/outreachandenrollment/. 3. This NoA provides outreach and enrollment (0/E) supplemental funding to support outreach and enrollment assistance activities, including the hiring of new 0/E assistance workers at a minimum of 1,0 FTE within 60 days of award. All 0/E supplemental funds are to be used to support new outreach and enrollment capacity and not supplant existing resources. Due to the availability of resources, your funding amount has been adjusted by $4,000 and consistent with Health Center Program statutory distribution requirements, adjustments have been made to your FY 2013 program allocation. These adjustments do not reduce your FY 2013 target funding level nor impact your FY 2013 funded activities. 4. Broad-reaching communication efforts, such as mass mailings, may be used to announce an 0/E event to be hosted by the health center and/or the availability of the health center as a location where 0/E assistance is available. These communications must also be reasonable and consistent with the intent of the 0/E supplemental funding opportunity and must not promote the health center or its services. In addition, to the extent possible, 0/E messages to be used, in whole or in part, to describe the purpose of 0/E activities and/or new affordable insurance options should rely on pre-approved federal and/or state materials, 5. The grantee should review, and update as needed, Form SC: Other Activities "Non-Clinical Outreach" so that activities/locations align with funded outreach and enrollment activities. See PIN 2008-01 'Defining Scope of Project and Policy for Requesting Changes,' pp.7-9, for additional guidance (http://bphc.hrsa.gov/policiesregulations/policies/pin200801.html).
