C-22-25-016-X-02 Amendment No. 2 STF FY26 Rapid Rehousing Page 1 of 4 AMENDMENT NO. 2 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND SAVE THE FAMILY FOUNDATION OF ARIZONA I. Maricopa County (“County”) administered by its Human Services Department and Save The Family Foundation of Arizona. (“Subrecipient”) entered into a financial agreement (“Agreement) on or about August 25, 2024. The original Agreement term was July 1, 2024, through June 30, 2025. The purpose of the Agreement is to provide housing stabilization services and short-term and/or medium-term rental assistance and case management services to help individuals and families at risk of or experiencing homelessness. The County provided the Subrecipient with $52,390.00 in U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) Program Year/Fiscal Year 2024/2025 funds under Assistance Listing Number (ALN) 14.231, and $433,521.00 in Program Year/Fiscal Year 2024/2025 funding through an Intergovernmental Agreement (IGA) with Arizona Department of Economic Security (DES) under IGA DI23-002395/ C-22-23-107-X- 00. The original Agreement total funding amount was $485,911.00. The County and the Subrecipient may be referred to individually as the “Party” or collectively as the “Parties.” The parties executed Amendment No. 1 on or about September 11, 2025. The Amendment revised the Agreement to reflect new total Agreement funding in the amount of $385,658.00 for Fiscal Year 2026, funded through the County’s IGA DI23- 002395 with DES, revised the Contract expiration date through June 30, 2026, revised and replaced required Paragraphs in Section 1, Section 2 and in Section 3 (Work Statement) replaced the budget Paragraph 2.0 and updated Points of Contact. II. The Parties now agree to modify the Agreement as authorized by Section 1 (General Provisions), Paragraph 4.0 (Amendments) of the Agreement as follows: A. Revise Agreement Amount on Page 1 to increase funding by $67,959.57, for a new total Agreement funding amount of $453,617.57, for Fiscal Year 2026 (FY26). B. Extend the Agreement Expiration Date on Page 1 through June 30, 2027, for the budget period of July 1, 2026 through June 30, 2027 (Fiscal Year 2027). Costs incurred in the FY27 budget period shall only be eligible for reimbursement under this Agreement subject to County’s receipt of funding, once funds have been added to the Agreement by Amendment. C. In Section 3 (Work Statement) revise and replace the following: 1. In Paragraph 2.0 (Budget) replace with: C-22-25-016-X-02 Amendment No. 2 STF FY26 Rapid Rehousing Page 2 of 4 2.0 BUDGET Activity Budget Fiscal Year Rapid Rehousing $453,617.57 FY2026 Total $453,617.57 2.1 The County shall provide the Subrecipient an amount not-to-exceed four hundred fifty-three thousand six hundred seventeen and fifty- seven hundredths ($453,617.57) subject to terms of this Agreement and availability of funds for the FY26 Budget period. Funding is provided to the County through an Intergovernmental Agreement (IGA) with Arizona Department of Economic Security (ADES) (Agreement #D123-002395) for Fiscal Year 2026. This Agreement amount constitutes the County’s entire participation and obligation in the performance and completion of all work to be performed under this Agreement 2.2 Reimbursement to the Subrecipient is in accordance with actual allowable costs incurred not to exceed the service reimbursement ceiling as stated in the Itemized Service Budget in 2.4 2.3 Funding for this Agreement is through the following published programs in the Federal Register by the executive departments and agencies of the Federal Government of the United States under Assistance Listing Number (ALN): $453,617.57 - 14.231 Emergency Solutions Grant 2.4 Itemized Service Budget: 2. III. The Agreement is amended to incorporate the changes contained in this Amendment No. 3. All other terms and conditions of the Agreement remain in full force and effect as executed by the Parties. This Amendment No. 2 is subject to and incorporates the provisions of A.R.S. § 38-511. IV. The Parties have authorized the undersigned to execute this Amendment No. 2 on their behalf and it shall be effective upon approval and signature by both Parties.
