REQUEST FOR COUNCIL ACTION

ACTION:  Adopt a resolution accepting a grant from Contexture in the amount of $70,000 to be used 
for Community Health Worker Integration Pilot program and authorizing the Mayor to execute the 
CommunityCares Access Agreement. (Resolution No. R2026.01)
FISCAL IMPACT:  The amount of the grant award from January 1, 2026, through January 31, 2027, 
is $70,000.  This grant award will supplement the Community Health and Human Services' existing 
General Funds appropriation. Sufficient budget appropriations for expenditure of this grant were 
authorized in the Governmental Grants Fund (Fund 46) as part of the fiscal year 2025-2026 adopted 
budget.
RECOMMENDATION:  Adopt Resolution No. R2026.01.
 
BACKGROUND INFORMATION:  As part of the City of Tempe Opioid Response Plan, the 
Community Health and Human Services Department hired Community Health Workers (CHWs) to work 
alongside Tempe Fire Medical Rescue in addressing high utilizers of emergency medical services. The 
CHWs focus on individuals with elevated risk of opioid misuse, opioid overdose, and those living with 
Opioid Use Disorder (OUD) in addition to other individuals with recurring human service needs. CHWs 
will provide linkages to care, referrals for treatment and recovery support, and navigation of health and 
social service systems.
Through the partnership with Contexture, CHW staff will use the CommunityCares software platform to 
ensure service referrals are monitored and completed and individuals are connected to the appropriate 
community resources. This will enable the CHW staff and its community-based partners to improve 
coordination of care and develop a model for CHW-delivered services, creating a financially sustainable 
services that extend beyond short-term grant support.
