A RESOLUTION OF THE CITY COUNCIL OF THE CITY 
OF TEMPE, ARIZONA, ACCEPTING A GRANT IN THE 
AMOUNT OF $70,000 FROM CONTEXTURE TO BE USED 
FOR 
THE 
COMMUNITY 
HEALTH 
WORKER 
INTEGRATION PILOT PROGRAM AND AUTHORIZING 
THE MAYOR TO EXECUTE AN AGREEMENT WITH 
CONTEXTURE TO ACCESS THE COMMUNITYCARES 
PROGRAM. 
WHEREAS, as part of the City of Tempe’s Opioid Response Plan, the Community Health 
and Human Services Department hired Community Health Workers (CHWs) to work alongside 
Tempe Fire Medical Rescue personnel in addressing high utilizers of emergency medical services; 
and 
WHEREAS, the CHWs will focus on individuals with elevated risk of opioid misuse, 
opioid overdoses, and those living with Opioid Use Disorder in addition to other individuals with 
recurring human service needs; and 
WHEREAS, the CHWs will provide linkages to care, referrals for treatment and recovery 
support, and navigation of health and social service systems; and 
WHEREAS, the City plans to partner with Contexture, a nonprofit organization that 
provides strategic, technical and administrative support to communities committed to advancing 
health through information sharing, and use its CommunityCares software platform to ensure 
service referrals are monitored and completed and individuals are connected to the appropriate 
community resources; the partnership with Contexture will enable the CHWs and their 
community-based partners to improve coordination of care and develop a model for CHW-
delivered services, creating a financially sustainable service system that extends beyond the short-
term grant support. 
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY 
OF TEMPE, ARIZONA, hereby:
1.
Approves of and authorizes the acceptance of a grant in the amount of $70,000 from 
Contexture to be used for the Community Health Worker Integration Pilot Program; and 
2.
Authorizes the Mayor to execute the CommunityCares Access Agreement and 
other relevant documents with Contexture to obtain access to the CommunityCares software 
platform; and 
3.
Authorizes the City Manager (or designee) or the Community Health and Human 
Services Director (or designee) to take such further actions and execute such additional documents, 
including amendments, as are necessary to implement the grant and agreement and carry out the 
purposes of this Resolution.
