The SES Committee has fully approved your request for Fotokite Camera at a cost not to exceed for $24,622.68. IMPORTANT NEXT STEPS FOR REIMBURSEMENT Step 1: Order Equipment-Within 60 days of this letter (by February 17, 2025) Step 2: Go to www.100club.org Complete the SES Questionnaire Report and Upload Documents: A copy of the check used for the purchase Paid invoices that clearly show payment was made and a zero balance remains. Each invoice must be signed and dated by the head of the agency or authorized designee. A signed letter confirming the items were received in satisfactory condition, signed by the authorized person for the agency. The 100 Club will verify that expenses match the originally approved request. Reimbursement will not exceed the awarded amount or the invoice total.
