This addendum is required only if Client needs access to The Work Number 
database. The Work Number is an employment verification service provided by 
TALX Corporation, a wholly owned subsidiary of Equifax. In accordance with 
Justifacts agreement with Equifax to provide The Work Number information to end 
users, Justifacts is required to have the following information on file for any Client 
accessing The Work Number information. 
The Work Number may be used by Justifacts in the course of completing a background check on 
applicants/employees (Consumers). Data from the service may only be used to verify a 
Consumers employment status ("Employment Verification") or income ("Income Verification") 
for employment purposes.

An Employment Verification includes the Consumer's (i) employer name, (ii)
employer address, (iii) employment status, (iv) most recent hire date, (v) total
length of service, and (vi) position title, where available.
•
An Income Verification includes data provided in an Employment Verification, plus
the most recent two years payroll data.
Work Number Addendum I Justifacts Credential Verification, Inc.

The Work Number Addendum 
This addendum to the Service Agreement (Agreement) between Justifacts Credential Verification, Inc. 
(Justifacts) and City of El Mirage  (Client) is effective upon date of Client signature below. 
Justifacts and Client do wish to modify the terms of the Agreement as follows: 
In order to meet the requirements for obtaining an employment verification from Equifax, Client shall: 
1.
Client agrees that any data originating from Equifax will be requested only for Client's exclusive
use and held in strict confidence except to the extent that disclosure to others is required or
permitted by law. Only designated representatives of Client will request Equifax data on Client's
employee's/applicant's, and Client's designated representatives are forbidden from obtaining
Equifax data on themselves, associates or any other persons except in the exercise of their
official duties. Client will not disclose Equifax data to the subject of the request except as
permitted or required by law, but will refer the subject to Justifacts.
2. Client agrees to hold Equifax and all its agents harmless on account of any expense or damage
arising or resulting from the publishing or other disclosure of Equifax data by Client, its
employees or agents contrary to the conditions of section 1 or applicable law.
3. Client understands that Equifax does not guarantee the accuracy or completeness of Equifax data
and Client releases Equifax and Equifax's agents, employees, affiliated credit reporting agencies
and independent contractors from any liability, including negligence, in connection with the
provision of Equifax data and from any loss or expense suffered by Client resulting directly or
indirectly from Equifax data. Client covenants not to sue or maintain any claim, cause of action,
demand, cross-action, counterclaim, third-party action or other form of pleading against Equifax,
Equifax's agents, employees, affiliated credit reporting agencies, or independent contractors
arising out of or relating in any way to the accuracy, validity, or completeness of any Equifax
data.
4. Client agrees that all requests for Equifax data are only for the permissible purpose agreed to
under Justifacts "Service Agreement - Agreement to Abide by the Fair Credit Reporting Act".
Additionally, all other Client requirements under the FCRA, as listed in the Justifacts "Service
Agreement - Agreement to Abide by the Fair Credit Reporting Act" apply to all Equifax data.
Equifax may periodically conduct audits of Clients regarding compliance with the FCRA and
other certifications within this agreement. Client gives its consent to Equifax to conduct such
audits, with reasonable notice, and agrees that any failure to cooperate fully and promptly in the
conduct of any audit may result in suspension of access to Equifax data. Audits will consist of
requests for documentation related to a specific request for Work# information. If Equifax
terminates access due to conditions in the preceding sentence, Client (i) unconditionally releases
and agrees to hold Equifax harmless and indemnify it from and against any and all liabilities of
whatever kind or nature that may arise from or relate to such termination, and (ii) covenants it
will not assert any claim or cause of action of any kind or nature against Equifax in connection
with such termination.
