ALL FUEL INVOICES ARE NET 10 DAYS
*A Damage Waiver will be charged at 15% of rental rate, if no Certificate of Insurance is provided within 20 days of delivery, or the end of the rental whichever comes first.
24 hour fuel service is available, please call 623-937-1719 for current fuel prices. If you do not choose Gen-Tech to be your fuel provider, there will be a fuel service
charge of $6.50 per gallon if the equipment is not returned full of the proper fuel. A clean-up fee of $75 per hour will be charged for equipment needing excessive cleaning.
Our equipment requires maintenance every 250 hours and is the responsibility of the customer, unless otherwise scheduled with GEN-TECH .
Prices contained in this estimate are not valid after 30 days of estimate date. There will be an additional 3% convenience fee added if paid by credit card.

Dear Mr. Slater, and Mr. Evaets:

Thank you for submitting a response to Invitation for Bid No. BPM002560. | am pleased to inform
you that your company's offer has been selected for award. The initial contract term of one year
shall begin on August 1st, 2020 and terminate on August 1st, 2021. Four (4) additional terms are
available for renewal.

The procurement file for this solicitation, including evaluation documents and resultant contract,
will be available shortly via the State's e-Procurement system, Arizona Procurement Portal
(APP).

In order to complete the contract award process and in accordance with the Special Terms and
Conditions, your company is required to submit a Certificate of Insurance. The certificate of
insurance (COI) shall indicate that your company is in compliance with insurance requirements
contained in the contract. Please submit your certificate of insurance to me as soon as possible.
Once the COI has been received, the Offer and Acceptance form will be signed by the State

Evaluation of Offers: Offers received from Arizona Generator, Bud Giffin, Cummins and WW Williams were evaluated in

Negotiations/Clarifications: Negotiations/Clarifications were conducted with all offers received with clarifications to their
original offers.

Award Recommendation: In order to meet the requirements of the WSB unit and facilities, recommendation is made to
award contract to both Arizona Generator and Cummins.

e Both scored “Meets” for the Methodology requirements

e Arizona Generator had a slightly higher score than Cummins on the Experience and Capacity requirements.
Primarily due to their previous experience working with WSB. Cummins scored high on the pricing for hourly labor
rates and for the maintenance of the existing generators for facilities.

Additional negotiations are not recommended as the above offeror's proposals, as submitted, represent the best value for
the State.
