BUILDING & SAFETY 1. No comment. ECONOMIC DEVELOPMENT 2. No comment. ENGINEERING 3. See attached memorandum. FIRE DEPARTMENT 4. No comment. PLANNING & ZONING 5. The applicant shall in consent to the following zoning stipulations. 1. The right to a use and occupancy permit shall be contingent upon the fulfillment of all general and special conditions imposed by the Conditional Use Permit procedure. 2. All the special conditions shall constitute restrictions running with the land and shall be binding upon the owner of the land, his or her successors or assigns. 3. All conditions specifically stated under any conditional use listed in Zoning Code Chapter 154 shall apply and be adhered to by the owner of the land, his or her successors or assigns. 4. All the special conditions shall be consented to in writing by the applicant by adding these stipulations to the site plan. 2 5. The proposed Collocation for wireless communication provider shall not generate noise, vibrations, and odors that may become a nuisance to the public. POLICE DEPARTMENT 6. No comment. PUBLIC WORKS 7. These items will have to be addressed prior to the Planning and Zoning Meeting. 1. None The below items will have to be addressed during the Construction Plan Submittal. 1. The “Notice of Eligible Facilities Request” document provides deadlines of February 3rd and March 5th for notice of incomplete application and issuance of approval, respectively, as if the plans were submitted to the City on January 4th. These plans were actually submitted on March 9th. 2. The deed provided is not the most current. Site Plan 3. Sheet T-1, Site Information - The Zoning District that the site lies within is Zone EI. 4. Sheet A-1.2 – The site lies within Flood Zone X (Shaded) which has a different description than what is shown in Note #3. 5. Sheet A-1.2 & E-1 – The information in the Utility View is difficult to read. An aerial view may be better. 6. Sheet A2.1 – Show the height/elevation of the ice bridge? Will this be visible from El Mirage Road? 7. Sheet A-2.2 – This is labeled as the north elevation but it appears to be a copy of the east elevation. 8. Sheet A-3 & A-4 - What is the elevation of the highest point of the equipment? Is the HVAC cabinet shown on Sheet A-4 the same as the equipment cabinet shown on Detail 1 of Sheet A-3? If so, the cabinet is shown as 73 inches tall. Will this be visible from El Mirage Road? 9. Sheet E3-1 – Note 3 refers to Sheet A-1. Is this supposed to be A-1.2? 10. Sheet E3-1 – Detail 1 refers to an aerial-mounted transformer but there are no power poles or overhead lines in this area. 11. Sheet GN-2 – Site Activity Note 2 refers to the permits that may be necessary. An Engineering Permit from the City will be required. 2 12. Sheet GN-5 – This sheet title should be “RF Signage”. 13. Add City General Information, General Notes and Dry Utility Notes to the plans. 14. Show all existing utilities and easements within the proposed limits of disturbance. 15. The new fiber run from the tower site to the El Mirage Road right of way shall be located within an access/utility easement. Provide proof of the easement if it is existing or submit a legal description and exhibit for review if it is proposed. 16. If more than 0.10 acres will be disturbed, a Dust Control Permit from Maricopa County will be required. The above comments are meant to be general in nature and are not considered to be all inclusive. Additional comments will arise during the formal permit submittal.
