FY23-24 Transportation Services Contract

AMENDMENT NO. 3 
TO TRANSPORTATION SERVICE CONTRACT 
SERVICE AGREEMENT BETWEEN 
THE CITY OF EL MIRAGE AND NORTHWEST VALLEY CONNECT 
THIS AMENDMENT NO. 3 to the Transportation Service Contract service agreement is made 
and entered into this ________ day of _____________, 2023, by and between the City of El 
Mirage, an Arizona municipal corporation (“City”), and Northwest Valley Connect, an Arizona 
nonprofit corporation, on behalf of itself (“Contractor”).  
RECITALS 
A. WHEREAS, City of El Mirage entered into an agreement with Northwest Valley Connect 
commencing on July 1, 2020, for the City’s dial-a-ride transportation services (“Agreement”) for 
an initial contract period of twelve (12) months, until June 30, 2021.  
B. WHEREAS, the Agreement provides for two (2) additional two (2)-year extension periods in 
accordance with Section 3.1, TERM AND EXTENSION of the Agreement.  
C. WHEREAS, City desires to exercise the second two (2)-year period to extend the Agreement 
until June 30, 2026.  
D. The City of El Mirage is authorized and empowered by provisions of the City Code to execute 
agreements for professional services by and through its City Manager;  
AGREEMENT 
NOW, THEREFORE, in consideration of the mutual promises and obligations set forth herein, 
the parties hereto agree as follows:  
1. In accordance with Section 3.1, TERM AND EXTENSION of the Agreement, the term of the 
Agreement shall be extended an additional two (2) years through June 30, 2026.  
2. All terms, conditions and provisions of the Agreement shall remain unchanged and apply 
during the renewal period unless otherwise amended. IN WITNESS WHEREOF, the parties 
have executed this Amendment as of the date and year first set forth above.

EXHIBIT “A” Addendum 
NORTHWEST VALLEY CONNECT  
Cost Estimate FY 23-24 
The cost estimate for FY23-24 includes an increase in accessible trip cost due to fuel and driver 
pay increases and to the senior bus program to include an extra run when there are more than two 
clients who use wheelchairs.  
A) 1,826 Estimated Accessible Trips 
1 to 7 miles (1,800 estimated annual trips) 
 
$ 36 per trip x 1,800 = $64,800 
8 to 15 miles (20 estimated annual trips) 
 
$ 48 per trip x 20 = $960 
16 to 25 miles (6 estimated annual trips)  
 
$60 per trip x 6 = $360 
26 miles plus   
 
Negotiable – not to be included in cost proposal.

B) 4,000 Authorized Rideshare Provider Trips  =  $74,000

Riders will be charged $2 for each one-way trip and consultant will be responsible for billing the
