El Mirage Road El Mirage, AZ 85335 RE: DRAFT - Dysart Road: Contract Modification for Construction Management Services Dear Mr. Christo, As requested, we have modified our scope of services to include the inspection and construction management services required to support the construction phase of this project. This work will be comprised of the following: • Based on an assumed 15 month construction schedule, we will attend up to sixty four (64) weekly construction progress meetings at the project site or City offices. It is presumed that one (1) EPS staff member would attend all the meetings and an additional EPS staff member would attend half of the meetings. EPS will conduct the meetings on behalf of the City staff. EPS will prepare meeting agenda, attend the meetings, and prepare meeting minutes. • Based on an assumed 15 month construction schedule, we will provide inspection services for the duration of construction activities. EPS will provide one (1) full time inspector at an assumed 20 hours per week for month 2 and 16 and 40 hours per week for months 3-15. Should construction activities require the support of a second inspector, the City will provide that additional support. • Review and approve change orders received from the Contractor. Approvals of change order will be contingent on receiving concurrence from City staff. • Review and recommend for approval to the City pay applications as submitted by the contractor. There will be 4 separate pay applications required for each of the following: Dysart Road, Olive Avenue/Microsoft frontage, TI Cold & the basin at Peoria/Dysart. • Review RFI’s, materials and other submittals with the support of the design engineer. • Review test results for compliance with specifications. The following assumptions have been made in association with this modified scope of work: • The inspector will provide services for no more than 20 hours per week for month 2 and 16 and 40 hours per week for months 3-15. 1130 N. EPS will review the as-built plans once submitted. We appreciate this opportunity to assist the City with the construction of this Dysart Road project.
