Page 1 of 2 HOMELESSNESS OUTREACH NAVIGATION SERVICES AMENDMENT BETWEEN THE CITY OF EL MIRAGE AND PHOENIX GOSPEL MISSION, INC. dba PHOENIX RESCUE MISSION AMENDMENT #1 THIS AMENDMENT dated this 1st day of July, 2023, amends the following items of the Homelessness Outreach Navigation Services agreement entered into between the City of El Mirage and the Phoenix Gospel Mission, Inc., dba Phoenix Rescue Mission, dated the 7th day of November, 2021. (referred to as “Agreement”) The following Sections amend those sections of the Master Agreement entered into November 7, 2021. Agreement 1. Term of Agreement This Agreement shall be effective as the date first set forth above and shall expire upon the expiration of contract ACON46519, unless terminated, cancelled, or extended as otherwise provided herein. The City will terminate this agreement when ACON36519 expires or when the agreement with the West Valley Homelessness Cooperative is finalized, whichever is sooner. All other terms of the Parties Homelessness Outreach Navigation Services Agreement remain unchanged and in full force and effect.
