ARIZONA COMPLETE HEALTH 
& 
MARICOPA COUNTY, ARIZONA 
 
 
WHEREAS, Maricopa County (“County”) and Arizona Complete Health (“AzCH”) (“Contractor”) have entered into 
an Agreement for the purpose of establishing collaborative protocol for effective communication, coordination, and 
continuity of care for individuals eligible for services provided by Arizona Complete Health who are also served by 
Correctional Health Services executed November 7, 2024 (“Agreement”).  
 
WHEREAS, County and Contractor have agreed to renew this Agreement. 
 
NOW, THEREFORE, the parties hereby agree to the following provision: 
 
The Agreement shall be renewed for a two-year term from November 7, 2025, and ending November 7, 2027.  
 
ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. If there is any conflict between the 
terms of the Agreement and this Amendment, in all such events, the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Amendment is executed on the date set forth below when executed by both Parties'
