RESOLUTION NO. R21-66
A RESOLUTION OF THE COUNCIL OF THE CITY OF
GLENDALE, MARICOPA COUNTY, ARIZONA, ENTERING
INTO AN AGREEMENT TO ACCEPT AND EXPEND FUNDS
FROM THE MOBILIZE AZ GRANT FROM BLUE CROSS BLUE
SHIELD OF ARIZONA, FOR THE MOBILEAZ SUBSTANCE
USE DISORDER PROGRAM IN THE APPROXIMATE AMOUNT
OF $30,000, ON BEHALF OF THE GLENDALE FIRE
DEPARTMENT
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF GLENDALE as follows:
SECTION 1. That the City Council of the City of Glendale authorizes the City Manager to
enter into the MOBILIZE AZ Grant Agreement, accept the grant award from Blue Cross Blue Shield of
Arizona, for the 2021-2022 MOBILIZEAZ Substance Use Disorder Program, in the approximate
amount of $30,000, on behalf of the Glendale Fire Department. Said agreement is now on file in the
office of the City Clerk of the City of Glendale.
SECTION 2. That the City Council of the City of Glendale authorizes expenditure of said
grant funds for the 2021-2022 MOBILEAZ Substance Use Disorder Program.
SECTION 3. That the City Manager or designee and City Clerk are authorized and directed
to execute any and all documents necessary to enter into said agreement and for the acceptance and
expenditure of said grant on behalf of the City of Glendale.
PASSED, ADOPTED AND APPROVED by the Mayor and Council of the City of Glendale,
Maricopa County, Arizona, this 22nd day of June, 2021.
