AMENDMENT NO. 2 
AGREEMENT FOR PRE-EMPLOYMENT POLYGRAPH EXAMINATION 
SERVICES  
(AZPE Inc., d/b/a Wells Polygraph Services, Contract No. C21-0669) 
 
This Amendment No. 2 (“Amendment”) to the Agreement for Pre-Employment Polygraph 
Examination Services (“Agreement”) is made this ______ day of ________, 2023, 
(“Effective Date”), by and between the City of Glendale, an Arizona municipal corporation 
(“City”) and AZPE Inc., d/b/a Wells Polygraph Services, an Arizona corporation authorized 
to do business in Arizona (“Contractor”). 
 
RECITALS 
 
A. 
City and AZPE Inc., d/b/a Wells Polygraph Services (“Contractor”) previously 
entered into an Agreement for Pre-Employment Polygraph Examination Services, 
Contract No. C21-0669, dated July 23, 2021 (“Agreement”); and 
 
B. 
The Agreement is in effect until July 22, 2023, with three (3) remaining one (1)-year 
optional extensions, renewable on an annual basis through July 22, 2026; and  
 
C. 
City and Contractor wish to modify and amend the Agreement subject to and strictly 
in accordance with the terms of this Amendment. 
 
 
AGREEMENT 
 
In consideration of the mutual promises set forth herein and other good and valuable 
consideration, the receipt and sufficiency of which are hereby acknowledged, the City and 
Contractor hereby agree as follows: 
 
1. 
Recitals.  The recitals set forth above are not merely recitals, but form an integral 
part of this Amendment. 
 
2. 
Term.  The term of the Agreement will be extended for another one-year term and 
shall expire on July 22, 2024, unless additional optional renewals are elected. 
 
3. 
Scope of Work. The Scope of Work remains unchanged. 
 
4.  
Compensation. The total compensation for this agreement is increased by 
$111,000, for a new not to exceed amount of $159,000 over the entire term of the 
Agreement.  Effective July 23, 2023, the price schedule will reflect the following rate: 
 
Pre-Employment Examination 
$220.00/Per Exam 
Specific Issue Polygraph Exam Services 
$220.00/Per Exam 
Less than 24 hour cancel/no show fee 
$25.00/Per Exam
