Description of Change:

Changes to the scope of work included the addition of the EB Union Hills Drive at 67th Avenue location which was
included in the original bid but not awarded. Contract increase will also include an additional $25,000 Owner's
Allowance. Contractor will furnish labor, equipment, materials, and all services required to make additions,
deletions, and changes, including changes to quantities and/or prices as outlined in attachment.

‘Supplemental Agreement Force Account Request, final payment shall be made as stipulated in the Standard Specifications and its
supplements upon completion of said work.
