                       TOTAL STUDENTS/INSTRUCTORS: 237
HOST will be the City of Glendale Fire Department in accepting the Fiscal Year (FY)
2023 Assistance to Firefighters Grant (“AFG Grant”) on behalf of itself and PARTNERS. The
AFG Grant is utilized to implement NFPA 1021, Fire Instructor I and Fire Officer I&II/
Leadership for the purpose of protecting the health and safety of the public and first responder
personnel against fire and fire-related hazards. The total amount awarded plus the participants
15% match totals $2,705,348.73 ($11,414.97 average per firefighter).

As part of the requirement for the AFG Grant, HOST and PARTNER are required to sign
all documents being provided by either the HOST Fire Chief or Authorized Representative.
HOST and PARTNERS agree to their portion of the 15% match.
The total approved budget per the grant award is $2,705,348.73, with $2,352,477.15
awarded in Federal funding and a required match contribution of non-Federal funds equal to or
greater than $352,871.58. PARTNERS are required to submit a reimbursement request and
associated supporting documentation to HOST for submission to FEMA. All PARTNERS will
be reimbursed upon FEMA reimbursement to HOST agency.
PARTNER will pay all up front for their personnel costs and submit the required payroll
reports to the HOST. The HOST will submit all required documents for FEMA reimbursement
HOST agency will pay all contractual costs up front associated with the student and or
instructor. These costs include, but may not be limited to, the per student expense for course
materials and testing fee from the Arizona Center for Fire Service Excellence. When a
PARTNER submits all necessary payroll paperwork to the HOST for FEMA reimbursement, the
HOST will submit for all pre-paid, per student contractual expense for FEMA reimbursement.
When the HOST receives approval and identified reimbursement amounts from FEMA,
HOST will reimburse PARTNER.
HOST and PARTNERS acknowledge and agree that all respective activities related to the
AFG Grant process and administration shall comply with federal Environmental Planning and
Historic Preservation Compliance (EHP) regulations, laws, and Executive Orders as applicable,
and any other requirements set forth under the AFG Grant website at
https://www.fema.gov/welcome-assistance-firefighters-grant-program.
IN WITNESS WHEREOF, HOST and PARTNERS have executed this MOU as of the
dates written below.
